I think the most common meaning is when you say you don't have a boyfriend or girlfriend more. So, you broke the relationship up. It can be also used when you break up a relationship between friends.
Ex: We had been having some problems, he had never have time to talk about this and we decided to break up. We broke up!
You can use that when you are talking on the phine and you can't hear the person's voice very well, then, you could say You are breaking up.
Another use is when you want to say something about the weather.
For example: "When the storm breaks up, the weather improves."
The other meaning is when you are fighting and you break it up, so you bring an end to the fight.
